don't know why it does that sometimes, but you can always go back in and delete the second one. Might be double clickin the submit button, but I'm not sure. Maybe Admin can help with answerin this.
Lilly is right. The usual cause is clicking the submit button twice. Give it a few seconds before you click it again if it doesn't go through quickly the first time.
