Yes, I know personally of a complaint that was not adequately investigated. I will not get into the specifics because it has been brought to the attention of at least two senators and we want to see if it will be looked at again.

No written response was ever issued for the complaint. We were told how it was handled. It was mishandled in precisely the way described in the OIG report.

Come to think of it, I know of another that didn't even get looked at.
